He was predeceased by: his parents, Catherine Clark Dame and Emon Sidney Dame; and his brother Paul C. Dame. He is survived by: his wife Carolyn Todd Dame; his children, Stephen Dame (Megan) of Slaughters, KY, Teresa Blain of Madisonville, KY and Vicky Scott (Patrick) of Manitou; and his brothers, Dennis Dame (Bonita) of Manitou and Danny Dame of Manitou. He is also survived by eight grandchildren; six great-grandchildren; and several nieces and nephews.
