He was predeceased by: his parents, Lola Collins (Lawson) and Leonard Collins; his wife Alice Denise Collins (Henson); his grandson Isiah Gage Collins; and his sisters, Shirley Sizemore, Elsie Sue Eversole and Elizabeth Collins.
He is survived by: his children, Crystal Michelle Henson (Rocky Namau) of Manchester and Anthony Clyde Collins (Whitney Bowling) of Manchester; and his siblings, Randy Collins (Kenya) of Middletown, OH, Denver Collins (Della) of Elizabethtown, KY, Leonard Collins, Jr. (Bernice) of Hector, Gilbert Lee Collins of Manchester and Willie Mae Collins of Manchester. He is also survived by five grandchildren.
