He was predeceased by: his parents, Myrtle Sizemore Smith O'Brien and Hardin Smith; his daughter Amy Smith; his siblings, Eulene Smith and Leonard Smith; and his nephew Rick Smith.
He is survived by: his wife Debbie Smith of Burning Springs; his children, Kim Bush of Burning Springs and Jason Smith (Crystal) of Burning Springs; his grandchildren, Lindsey Smith (Jamie) of Burning Springs, Brittany Bush (Hunter) of Georgetown, Nick Smith and Preston Bush of Burning Springs; and his great grandchildren, Amelia Smith, Avery Smith, Adeline Smith and Blakely Bush of Georgetown.
