He was predeceased by: his parents, Kelly Collins and Faye Collins (Robertson). He is survived by: his daughter Kellie Collins; his grandchildren, Wyatt Perkins, Waylan Perkins and Wade Perkins; and his brothers, Randy Collins and Gary Collins.
The family requests expressions of sympathy take the form of donations to the funeral home to help offset funeral expenses.
