He is survived by: his children, Jonathan Cook (Pauline) of Crum, WV and Crystal Marcum (Wayne) of Louisa, KY; his siblings, Freeman Cook of Debord, KY, Willis Cook of Inez, KY, Ellis Cook and Darlene Cook of Lowmansville, KY.; and his grandchildren, Joshua Marcum, Alana Cook, Ryleigh Cook and Dillon Waller. He is also survived by several nieces and nephews.
