He is survived by: his spouse Jill Graf Smith; his daughter Samantha Hazelwood (Johnny) of Hazard; his stepdaughters, Mindy Swonger (Tom) of Massillon and Brooke "Mia" Parks of Miami; his siblings, Opal Smith, Charlie Smith, Sue Goss (Lloyd), Haven Smith (Connie), Steven Smith (Rose) and Agness Smith; and his grandchildren, Ellie, Brantley, Grayson, Nathan, Kelsi, Kym, Adrian, Marcy and Vanessa.
