He was predeceased by: his wife Joyce Jacobs Bentley; his parents, Jakie Marion Bentley and William T.; and his sister Kyla Lee Hedrick. He is survived by: his children, Beverly Jones (Roger) of London, Donna Hodge (Darrell) of London and Lorie Stines (Joseph) of London; his grandchildren, Lesley Anne Jones-Frazier (Mike) of London, David Edward Jones (Megan Vanhook) of London, Jacob Canton Stines (Jessica) of London, Lauryn Kayla Stines of London and Natalie Nicole Hodge of London; and his great grandchildren, Bailey Raeanne Frazier, Rayse Michael Frazier, Madison Aubrey Kate Vanhook, Sienna Marie Jones and Bentley Curtis.
In lieu of flowers, the family requests donations be made to the North Laurel High School Band, 1300 East Hal Rogers Pky., London, KY 40741 by putting "In memory of Jack Bentley" on the memo line.
