He was predeceased by: his parents, Clarence Johnson Hayes, Sr. and Willie Mae Spencer; his stepmother Julia Howard Hayes; his son Courtney Eugene Embry; his grandson Chris L. Carter; his sisters, Juanita Mason Pryor, R. Victoria Hayes and Julia Ann Hayes Clay; and his mother-in-law Alberta Davis Smith Johnson.
He is survived by: his wife Carolyn Wilson Embry; his brother Clarence Johnson Hayes, Jr.; his brother-in-law Kevin L. Johnson; his sister Margaret Hayes of Gary, IN; his children, Vondale Harris Ward and Sunserria Irvin Watson; his niece Stephanie Renee Davis; and his cousins, Ella M. Bosley, Diana Kimball, Dean Hayes and Phyllis Alcorn (James). He is also survived by several grandchildren; great-grandchildren; and great-great-grandchildren; many nieces and nephews.
