He was predeceased by: his son Joseph Anthony Cook (Tony). He is survived by: his wife Mary Therese Cook; and his children, Joshua James Hager, Harley Lane Hager, Shawn Brock, Angie Michelle Fallis, Sabrina Hager, Misty Burbbage and Samantha Renee.
A celebration of life will be held on Sunday, February 23rd 2025 at 6:00 PM at the Proud Mary's (9079 Old Richmond Rd, Lexington, KY 40515).
