He is survived by: his wife Jackie Zambrano-Mantz; his daughters, SueAnn Unger (Nielsen), Sarah Scott (Brandyn), Arianna Biggs (Joseph), Sara Mantz, Rachel Sender and Christine Santillan; and his grandchildren, Briella Unger of Georgetown, KY, Ethan Scott of Riverview, FL, Lily Scott of Riverview, FL, Autumn Scott of Riverview, FL, Kilian Biggs of Knoxville, TN, Landyn Perkins of Lexington, KY and Caleb Sender of Lexington, KY.

In lieu of flowers, memorial donations may be given to The Ronald McDonald House of Lexington, KY.
