He is survived by: his parents, Matt Holbrook and Shauna Crowe (Kevin); his siblings, Nevaeh Holbrook and Coy Middleton; his stepbrother Billy Middleton; his grandparents, Mike, Nancy Holbrook, Larry Brooks, Carol Brooks, Mona Brooks and Jim Morgan; his stepfather John Middleton; his girlfriend Katherine "Katie" Adams; and his aunts and uncles, Shannon Brooks, Larry Brooks, Adam Holbrook (Alison) and Jason Holbrook. He is also survived by numerous cousins, friends.
