He was predeceased by: his parents, Gene Ballard and Edith Ballard; and his sister Cathy Jones. He is survived by: his wife Donna; his son Blake; and his sister Kelly Sanders (Scott). He is also survived by nieces, nephews and extended family.

In Lieu of flowers, contributions may be made to the American Heart Association, Bluegrass Care Navigators, or the Lexington Humane Society.
