He is survived by: his mother Ruth Martin Caywood; his siblings, Joseph John Caywood and Mary Elizabeth Hamlett (Kenneth); his children, Andrew Martin Caywood, Sarah Hammond Caywood, Tricia Dawn Akers and Brelee Shae Smith; and his granddaughter Josephine Caywood.

In lieu of flowers the family suggests donations in David's name to the Catholic Action Center of Lexington, 1055 Industry Dr., Lexington KY 40505.
