He was predeceased by: his father Richard James Dake; and his brothers, Richard Joe Dake and Bryan James Johnson. He is survived by: his mother Helena Johnson (JJ); his daughters, Jamie Marie Dake and Alisha Michelle Dake; his siblings, Nathan Kyle Shane Dake and Sammy Dake; and the mother of his children Heather Morrow (Preston). He is also survived by extended family and many friends.

In lieu of flowers, donations can be made to Heady-Radcliffe Funeral Home to help cover the funeral expenses.
