She was predeceased by: her husband Samuel Forest Hooks; her parents, Harry Cook and Elizabeth Baldwin Cook; her daughter Margaret Jane Hooks Rogers; her granddaughter Tammy Leigh Boyd; her siblings, Walter Cook, Richard Cook, Rev. Robert Lyn Cook, Wayne Cook, Charles Cook, Artie Cook Sadler and Ruth Cook Nall; and her son-in-law James B. Boyd.
She is survived by: her daughter Sarah Boyd of Kuttawa; her grandchildren, Michele Jasis (Byron) and Craig Rogers (Sarah); her great grandchildren, Jessica Eaves (Ryan), James Jasis (Julie), Aeydon, Jasper Eaves and Emersyn Jasis; and her sister Betty Bridges of Frankfort. She is also survived by several nieces and nephews.
