He is survived by: his wife Shirley Webb Polly of Jenkins; his daughter Lisa Dawn Johnson (Teddy) of Jenkins; his grandson Tyler "Little Buddy" Collins of Jenkins; and his siblings, James "Jojo" Polly (Melinda) of McRoberts and Ruby Cook of Marion. He is also survived by nieces and nephews.
