He was predeceased by: his son David Nelson Willoughby; and his siblings, Myrtle Drake, Alvin Willoughby, Joe Pete Willoughby and Allen Willoughby.

He is survived by: his wife Bonnie Jean Hamilton Willoughby of Jeffersonville; his grandchildren, Andrea Dawn Willoughby and Jared Tyler Willoughby; his great grandchild Steven Tyler Willoughby; and his siblings, Margie McCarty (Mitchell), Linda Lukehart (Larry), Stan Willoughby, Wendell Willoughby (Karen) and Oliver "Red" Willoughby.
