He was predeceased by: his father David Ray Haddix. He is survived by: his mother Phyllis Perkins (John) of Jackson, KY.; and his brothers, David Ray Haddix, Jr. of Jackson, KY and Timothy John Perkins (Georgia) of Georgetown, KY.. He is also survived by other relatives and friends.
