He is survived by: his wife Elaine Cornett Cook; his daughter Crystal Cook Profitt (Keven) of Crafts Colley; his brothers, Gary Cook of Isom, Terry Cook of Colson and Tim Cook of Knott County; his grandchildren, Cheyenne Profitt and Harley Cook (Morgan); and his great grandchild Gage Cook.
