She was predeceased by: her stepfather Dee Blevins; and her grandparents, Mary Ruth Chism and Herbert Chism. She is survived by: her husband John Mark Wade of Irvington, KY; her mother Barbara Blevins of Irvington, KY; her son John Cody Wade; her stepchildren, Jason Allen Ferrari and Tiffany Anne Ferrari of Florida; her stepgrandchildren, Trinity Taylor and Sebastian Riley Ferrari; her siblings, Bobby Reichmuth (Becky), Robin Reichmuth (Mary) of Meade County, KY and Carmon of Flaherty, KY; and her nieces and nephews, Amanda and DJ. She is also survived by extended family and friends.
