He was predeceased by: his parents, Thomas Marion and Eva Opal Lively (Peavler). He is survived by: his children, Janet Lynne Zanetis (Alex) of Louisville, Thomas Lively (Barbara) of Junction City, KY, Leonard S. Lively of Destin, FL and William H. Lively (Pauline) of Irvine, KY. He is also survived by seven grandchildren; six great-grandchildren.
