He is survived by: his wife LeRoy Couch of Hyden, KY.; his children, Thomas Couch (Edith) of Hyden, KY., Teresa Couch, Frankie of Hyden, KY. and Helen Beringer; and his grandchildren, Bridget Malone (Nathan), Amanda Couch (Tanya Colwell) of Hyden, KY. and Dale.
