He is survived by: his son Jason Lewis of Hyden; his siblings, Josh Lewis (Beth) of Hyden, Lucy Lewis of Hyden and Alechia Drue Lewis of Hyden; his nephews, Hunter, Christian and Alijah; and his niece Bella Lewis. He is also survived by nieces, nephews, other relatives and friends.
