He was predeceased by: his father Hurshel Maddox. He is survived by: his mother Doris Swearinger Maddox of Butchertown; his children, Kayla Jeffries (Chris) of Hustonville and Kyle Thomas Maddox of Hustonville; his close friend Marsha Salyers of Casey County; his siblings, Michael Maddox (Shirley Bachelor) of Butchertown, Allen Maddox (Kathy) of Harris Creek, Jo Ann Terrell of Butchertown and Peggy Appleby of Butchertown; and his pet Jasper.
