He was predeceased by: his brother Danny Middleton; and his grandparents, Ludy Middleton, Dave Middleton, Benny Howard, Melinda Howard and Etta Lee Howard.
He is survived by: his wife Mary Middleton of Hulen, KY; his son Rodney Middleton of Harlan, KY; his siblings, Estel Middleton (June) of Hulen, KY, Rosemary Perry (Lee Sam) and Sandra Davis (Donald); his nephew Frank Stevens Very; his niece Robin Lyndsey; his friends, Charles Drew and Ruth Drew; and also, Willie Johnson (Betty), Frankie Ellahi and Virginia Hatfield. He is also survived by nieces, nephews and other relatives and friends.
