He was predeceased by: his parent Thomas Hayes "Hazy"; his daughter Gail Marie; his grandson Edwin Salerno; and his siblings, Catherine Rose Mills, Charlie and William Robert.
He is survived by: his daughter Laura Salerno of Louisville; his stepdaughters, Cindy Hahn (Mike) and Kathy Coy (Dennis); his sisters, Mary Lou Bright (David) of Louisville, Hazel Ann Fowler (Bill) of Louisville and Theresa Thomas of Bardstown; his brother-in-law Mike Mills; and his son-in-law Joe Mayfield. He is also survived by one daughter-in-law, Janet Downs; several nieces and nephews.
