He was predeceased by: his parents, Alvin Leslie and Hallie Leslie; and his sister Glenda Leslie. He is survived by: his wife Donna; his son Glen Leslie (Shelley Newman) of Henderson, Kentucky; his granddaughters, Misty Dilback (Adam) and Dawn Forker (Kale); and his great grandchildren, Anna Dilback, Adelyn Dilback, Mary Claire Forker and Lofton Forker. He is also survived by cousins; and nieces and nephews.
