He is survived by: his wife Breanna Lynn Enix Polly; his daughters, Avery Lynn Polly of Harlan and Alyssa Gene Polly of Harlan; his mother Doris Daniels; his stepfather Shawn Daniels; his siblings, Michael, Cody, Charles Howard, Matthew Mays and Joann Howard of Pathfork; his grandparents, Ralph and Norma Barnes; and his father-in-law David (Tammy Enix).
