He is survived by: his wife Phyllis Baute Middleton; his children, Lora Murphy (Mike) and Scott Middleton (Liz); his daughter-in-law Dana Middleton; his sister Bonnie Turner (Parnell); and his grandchildren, Rachel Raleigh (Kenny), Jacob Middleton (Victoria), Robbie Middleton, C. J. Middleton, Dalton Middleton (Makayla) and Shayna Wilson (Brandon).
Per mandates set by the Governor of Kentucky, everyone attending must wear some type of face mask and maintain social distance.
