He was predeceased by: his mother Janice Middleton; and his brother Matthew Middleton. He is survived by: his fiancée Joanne Joseph; his son Douglas "Bub" Middleton II; his father McArthur "Mac" Middleton; his sister Debbie Davis (Rod); his nephew Jake Davis; and his niece Amy Hardy. He is also survived by several aunts, uncles, cousins and other relatives.
