He was predeceased by: his father Luther Lesco Lively; his siblings, Rebecca Smith and Carlos Lively; his brother-in-law Henry Hammonds; and his grandson Dustin Middleton.
He is survived by: his wife Nancy Turner Lively; his children, Roy Lively (Joyce), Jerry Lively and Teresa Abner; his sister Charlene Hammonds; his grandchildren, Fonda Creech, Kathy Rice, Summer Lively, Ashley Lively and Debbie Lively Couch (Michael); and his great grandchild Max.
