He is survived by: his parents, Peggy Griffith (Nathan) of Winslow, IN and John Thomas Phebus (Carolyn) of Madisonville, KY; his daughter Ann Michele Johnson (Kyle); his granddaughter Emma Elizabeth Johnson of Madisonville, KY.; his siblings, Mia Salter and Mark Phebus of Lake Beshear; and his nephews, Michael of Madisonville and Maxx Phebus of Madisonville.
