He was predeceased by: his father James Calbert Dame, Sr.. He is survived by: his wife Cynthia Dame of Hanson; his children, Kristi Dame Gaither (Steven) of Hanson and Kyle (Jill) of Madisonville; his grandchildren, Rhett Dame, Keelie Gaither and Ryker Dame; and his siblings, Teresa Benson (Bruce) of Slaughters, KY, Neal Dame (Karin) of Hanson and Scotty Dame of Slaughters.

Memorial contributions may be made to Slaughters Cemetery.
