He was predeceased by: his parents, Roy Coleman Forsythe and Mary Forsythe (Bruce); and his sister Cindy Messamore. He is survived by: his wife Wanda Forsythe (Skinner) of Gracey; his children, Freddie Forsythe, Jr. of Hammond, Ind., Yvonne Forsythe of Earlington and Becky Forsythe of Hammond, Ind.; and his siblings, Jamie Forsythe of Hammond, Bobby Forsythe of Dawson Springs and Debbie Russell of Hammond, Ind.. He is also survived by seven grandchildren; seven great grandchildren.
