He was predeceased by: his parents, John W. Mayhew and Marjorie D. Mayhew of Scottsville, KY.. He is survived by: his wife Suzanne Lyles Mayhew; his sons, Dr. Ashley Mayhew (Cara) of Glasgow and Wesley Mayhew (Nicole) of Connecticut; his grandsons, Tristin (Auna), Landon, Aiden, Aaron and Corey; his great grandchild Houston; and his brother-in-law Steve Lyles (Nancy) of Alabama. He is also survived by nieces, nephews and cousins.
