He was predeceased by: his parents, Andrew Howard and Viola Garrett Underwood; his former wife Lisa Calhoun Underwood; his stepmother Mayoma Underwood; and his sisters, Carolyn Underwood Norris and Donella Underwood Bowles.
He is survived by: his wife Loretta Underwood; his children, Chris Underwood of Glasgow and Cory Underwood of Glasgow; his nephew Ronnie Miller; his nieces, Marcia Hubbard (Miller) of Glasgow and Lacey Ball of Glasgow; his stepsisters, Linda Combs (Martin) of Glasgow and Becky Lee (Mike) of Glasgow; his stepsons, Wesley Dunaway (Kristian) of Hopkinsville and Stephan Dunaway (Leme) of Owensboro; and his pets, Cody of Cady, JJ and Arlo.
In lieu of flowers, donations can be made in honor of Jerry Underwood to Cerebral Amyloid Angiopathy (CAA) Research Program at Massachusetts General Hospital.
