He is survived by: his wife Polly Cummins; his children, Kathryn Crouch (Bryan) and Max Cummins; his brother Donald Cummins (Joyce); his grandchildren, Mikayla Cummins and Dustin Crouch (Shayla); his great grandchildren, Addalynn Grace Crouch, KingsLeagh Maree Crouch, Gentry Thomas Crouch and Blayne Allan Crouch; his nephew Jason Edward Cummins (Misti); and his niece Rena Beth Jarrell (Tony).
Visitation was held on Tuesday, December 3rd 2024 from 10:00 AM to 12:00 PM at the Metcalfe-Hennessey Funeral Home (301 E 4th St, Augusta, KY 41002). A funeral service was held on Tuesday, December 3rd 2024 at 12:00 PM at the same location.
