He was predeceased by: his parents, James William and Mabel Morgerson Perkins; his son Stephen Duane Perkins; his grandson Dwayne Allen Perkins; his brother James L. "Jimmy" Perkins; and his children-in-law, Mary Perkins and Doug Anderson.
He is survived by: his children, Paige Perkins Anderson, Kelli Renee Perkins and Bradford Allen Perkins; his daughter-in-law Beth Perkins; his grandchildren, April Michelle Clayton (Wylie), Phillip Duane Perkins (Constance), Sarah Kimberly Perkins, Brandon Paul Perkins and Austin Douglas Anderson (Taylor); his great grandchildren, Kayla Renee Ferguson, Logan Ray Clayton, Lynnlee, John, Luke Perkins, Kamryn, Lakynn Anderson, Cash, Carter, Price and Dawson Perkins; and his great great grandchildren, Emersyn and Braxton Ferguson.
In lieu of flowers, memorials may be made to the Alzheimer's Association.
