We are sad to announce that on July 31, 2023, at the age of 38, Phoenix I. O'Faery (Georgetown, Kentucky) passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She is survived by: her parents, John Wynn and Roxanne Hopper; her husband Joseph M. Osborne; and her son Aiden J. Ice.
