She is survived by: her parents, Deborah Clifton (Charles) and Paul Wade (Joan); her children, Jessica Dixon (Brian), Sean Forman, Tyler Forman and Katelynn Forman; her grandchildren, Jax Dixon, Acel Dixon and Hazel Forman; and her siblings, Robert Wade (Becky), William Wade (Debralee), James Wade (Brittany), Michael Wade (Stephanie) and Elizabeth Wade.
