He was predeceased by: his parents, Virgil Shoulders and Helen Rodgers; his grandparents, Jim and Rosa Mae Shoulders; his stepmother Vallie Shoulders; and his siblings, Pat Johnson, Sherry Stevens, Danny Joe Shoulders, Ronnie Shoulders and Jimmy Shoulders.

He is survived by: his children, Nikki Shoulders (Tanner) of North Carolina and Leslie Polacek (Andrew) of Portland, TN; his grandsons, Grant, Drew and Luke; and his siblings, Judy Link of Franklin, KY, Pam Hawkins of Bowling Green, KY, Doug Shoulders of Franklin, KY. and Scotty Shoulders of Franklin, KY..
