She was predeceased by: her parents, Marcellus Perkins and Lila Wayne Perkins (Smith); her siblings, Marcella Perkins and Jimmy Perkins; and her niece Angie Cook Stephens.
She is survived by: her daughters, Mika Anne Forshee (Jim) of Franklin, KY and Chrissy Cummings (Kelly) of Franklin, KY; and her grandchildren, Anne Reid Forshee, Kristopher Cummings and Bree Anslea Forshee. She is also survived by "Nanny" loved spending time with her family, especially her grandchildren.
In lieu of flowers, expressions of sympathy may be made to the American Cancer Society in Julianne's memory.
