He is survived by: his wife Faye Mayhall; his sons, Michael Wayne (Kim) and Lloyd Allen Mayhall (Ashley); and his grandchildren, Kayleigh, Jacob, Ashlyn, Brendon, Azuri, Ryan and Chase.
