He is survived by: his wife Nadine Howard; his sons, Michael Cook and Mark Cook (Beverly); his grandson Simon Cook; and his siblings, Arthur Cook and Cindy Mincks (Carlos). He is also survived by numerous nieces, nephews and friends.
