He was predeceased by: his wife Mary Ann Middleton. He is survived by: his children, Ann Schmidt (Jack) of Orlando, FL, David Middleton (Laverne) of Lexington, KY, Richard Middleton (Carol) of Ft. Mitchell, KY and John Middleton (Janet) of Erlanger, KY.; and his grandchildren, Dustin (Bridget Schmidt), Alex (Rachael Middleton), Kelsey (Kyle Middleton) and Joey (Will Middleton).
