He is survived by: his wife Maggie; his children, David Jr. (Valerie), Susan (Mike Verax) and Courtney Cook (Gage); his grandchildren, Melanie, Joey Minnis and Porter Dailey; and his siblings, John (Terry), Douglas (Mary), Steven (Gina), Ellisa Stout, Paul (Joyce) and Matthew (LaDonna). He is also survived by many nieces, nephews, cousins and friends.
