He is survived by: his daughters, Dr. Amy Kathleen Midkiff (Christopher) of Lexington, KY and Mary Elizabeth Haggard (Chad) of Wurtland, KY; his grandchildren, Matthew Haggard, McKenzie Haggard, Lindsey Midkiff, AJ Haggard and Jack Midkiff; and his brother Earl "Shorty" Boyles of Lexington, KY. He is also survived by extended family and friends.
