He was predeceased by: his parents, Mitchell Hazelwood and Mary Whitaker Hazelwood; his wife Peggy; and his siblings, Bethel, Eugene Hazelwood, Katherine McClellan and Virginia Hazelwood.
He is survived by: his children, Tommy Hazelwood (Fay) of McDaniels, KY. and Tammy Fuqua (Jeff) of Fordsville, KY.; his grandchildren, Patrick Hazelwood (Becka), Amy Taylor (Adam), Autumn Paul (Jesse), McKenzie Richards, James Brown (Astara), Scott Brown (Roxanne), Dustin Brown (Jamie) and Heather Ferry (Matt); his great grandchildren, Chloe, Emily, Olivia, Nora, Teresa Taylor, Jackson, Jace, Addison, Jay Paul, Damion, Serenity, Daxxin, Pailsey, Preston, Kaydin, Kayson Brown and Valerie Williams; his sisters, Helen Embry (Dorothy Embry) of Hardinsburg, KY and Many; and his niece Due.
