He was predeceased by: his parents, Johnny Middleton and Snobia Jones Middleton; and his siblings, Anna Belle McQueen, Flossie Smith, Dosha Wynn, James Lee, Henry and Jack Middleton.
He is survived by: his wife Susie Dean Middleton; his son Jerry Walter Middleton, Jr.; his siblings, Kerry Middleton of Washburn, TN and Jessie Perry (Arnold) of Tazewell, TN; and his aunt Ellen Risner. He is also survived by nieces, nephews, other family and friends.
With current mandates placed in effect by Governor Beshear, attendance is limited to 50% capacity and all guests must wear a face mask at all times while practicing social distancing.
