He was predeceased by: his parents, Samuel Uzera and Lula Alice Collins (Dungan); and his siblings, Harold David, John Alfred Collins, Doris Collins Henry, Betty Collins Shelton, Bessie Collins Shelton and Patricia Ann Collins.
He is survived by: his wife Helen Jean "Jeanie" Gipson; his children, Robert "Rob" Harold Collins of Eubank, Carrie Collins of Somerset and Jennifer Collins Depuy (Andy) of Georgetown; his grandchildren, Kimberly Nicole Cruz (Jose), Heather Collins, Ryan Collins, Jamison Collins and Alex Depuy; and his great grandchildren, Gohan Clayton Loveless, Brysyn Robert Portman and Augustine Leon Cruz.
In lieu of flowers, the family requests expressions of sympathy take the form of donations to the Shriners Hospitals for Children Medical Center at 110 Conn Terrace Lexington, Ky 40508.
